Trinity Hospice provides the coordinated services of a multi-disciplinary team of professionals and trained volunteers through a personalized plan of care. Under the direction of a patient's physician and the hospice medical director, the hospice team works together to provide physical, emotional and spiritual care to the patient and their family.
The Trinity Hospice team works with your religious representative, as requested, to ensure that the religious resources of your tradition are available during the terminal illness.
Additionally, Trinity Hospice is pleased to offer a special program of care for residents of long term care facilities, the hospice team provides state-of-the-art symptom management, as well as emotional and spiritual support for the residents and their families.
Who Is Eligible?
Trinity Hospice offers service to patients who have a terminal illness where the goal of treatment is pain management and symptom control with a limited life expectancy as diagnosed by the patient's physician. Hospice patients do not have to be homebound. Patients must be aware of their condition and reside within the service area.
Financial coverage is available through the Hospice Medicare Benefit, Medicaid, and many private insurance companies. No person shall be denied services based on age, race, national origin, disease, handicap, religion, gender, color or sexual orientation.

If you, a family member, or a friend need hospice care, contact your physician or Trinity Hospice. Once your physician has given approval, a hospice representative will arrange an in-home visit to discuss your individual needs and further explain the services available through our program.

Your support of Trinity Hospice allows families, friends and neighbors to choose hospice care for their loved ones. Volunteers are needed to assist with companionship services, office and clerical support, professional services and public education.
Charitable contributions can be made to the Trinity Hospice Foundation, Inc. for this organization. All contributions are tax deductible and assist to provide for comprehensive hospice care for the community.
