TOLEDO, OH (Toledo News Now) -

A Toledo News Now viewer emailed us a photo after she says she found a metal utensil inside a salad she ordered from an area Wendy's restaurant.

We've alert the Lucas County Health Department of the incident. The department promised to send an inspector to follow up.

The department found 12 violations at East of Chicago Pizza on Heatherdowns Boulevard. The facility's salad bar wasn't properly turned on to main safe temperature. Food items had to be tossed out because of that.  An inspector also found no soap and paper towels at the kitchen sink.  The department is also advising the restaurant to clean its floors, walls and ceilings.

The Tropical Smoothie Café in Maumee, the Burger King on South Byrne Road in Toledo and the Arby's on Navarre Avenue in Toledo all passed inspection with zero violations this week.

Four violations were found at the Dog House Pub and Grub on Lewis Avenue. The health department is reminding the restaurant all personnel in the kitchen considered "the person in charge" must be food safety certified.

Cleaning is recommended by the health department at The Distillery on Heatherdowns Boulevard. Also, the inspector didn't find soap and paper towels at either bar for proper hand washing.
